Does your NPA require the APRN to have a collaborative agreement with a physician? In the state of Virginia, Every nurse practitioner other than a nurse practitioner licensed by the Boards of Medicine and Nursing as a certified nurse midwife or a certified registered nurse anesthetist or a nurse practitioner who meets the requirements of subsection I shall maintain appropriate collaboration and consultation, as evidenced in a written or electronic practice agreement, with at least one patient care team physician (§ 54.1-2957. Licensure and practice of nurse practitioners, 2021). I feel as though NPs should have to collaborate with a physician because a physician will be able to perform the same roles as the NP, however they are educated in a greater detail than the NP. It is no difference than a CNA working with a RN. The CNA is able to do some of the tasks that a RN can perform. However, we are trained at a higher level and the explanation as to the why is taught. 2. Does your NPA require the APRN to have a prescribing agreement with a physician? In Virginia, NPs who do not have at least 5 years of full time clinical hours must have an agreement with a physician to prescribe medications. NPs in Virginia, must have over 1,000 hours of experience to 30 credit education hours to prescribe medications. I feel that NPs should have an agreement with a physician in order to prescribe medications to a patient. I believe it depends on the viewpoint that you are looking at. I do not think that it should be considered as if we do not know what we are doing. I look at it as a safety check for the patient. This way, you will have someone with more education and knowledge that you can use as a resource for the well being of the patient. N521 Advanced Pharmacology Assignment 1 Paper 3. Does your NPA permit APRNs to prescribe all classes (schedules) of medications? In Virginia, Code § 54.1-2957.01. states that Such nurse practitioner shall provide to the Boards of Medicine and Nursing such evidence as the Boards may jointly require that the nurse practitioner has entered into and is, at the time of writing a prescription, a party to a written or electronic practice agreement with a patient care team physician that clearly states the prescriptive practices of the nurse practitioner. Such written or electronic practice agreements shall include the controlled substances the nurse practitioner is or is not authorized to prescribe and may restrict such prescriptive authority as described in the practice agreement. Evidence of a practice agreement shall be maintained by a nurse practitioner pursuant to § 54.1-2957. Practice agreements authorizing a nurse practitioner to prescribe controlled substances or devices pursuant to this section either shall be signed by the patient care team physician or shall clearly state the name of the patient care team physician who has entered into the practice agreement with the nurse practitioner. With the opioid epidemic, I feel that there could not be enough checks and balances to help defeat this issue. I feel as though, we as practitioners could possibly become skewed with our viewpoint of a patient with a chronic illness. Having a physician to discuss a patients case and when or if their medications have reached a therapeutic level and if they should need to be adjusted at that point would benefit the patient. I am of the opinion that there can never be too many checks for safety. 4. Describe collaborative approaches to treating rashes across the lifespan. Should the CNP treat without a collaborator or consultant? Collaborating with a dermatologist on treating rashes would benefit the patient. There are several reasons that a rash could come about. In my opinion, it would be better to consult a specialist versus running unnecessary tests or prescribing unnecessary medications that may not work. In the long run, that may be costing the patient more out of pocket costs. Since Covid, several of my colleagues as well as myself have developed skin issues due to the consistent use of mask wearing and other PPE. Moreover, skin problems related to personal protective equipment (PPE) have been reported in health care workers; thought to be caused by the hyper-hydration effect of PPE, friction, epidermal barrier breakdown, and contact reactions. Aggravation of previous dermatitis has been noted in the wearers and also skin lesions ranging from contact and pressure urticaria or contact dermatitis, after prolonged contact with masks and goggles (Jagadeesan & Sarkar, 2020).
