Does the nurse have to sit in front of a computer all day? Far from it! Assignment: Topic 1 Nursing Informatics “Nursing Informatics is not new, but it certainly is NOW! Technology is dramatically altering the ways in which we diagnose, treat, care for and manage patients. It is our past and it is our present” (Cleveland Clinic, 2010, 1). Nursing Informatics and the Nursing Process According to Sandy Savage (2010), Associate Director of Informatics at Kingman Regional Medical Center (KRMC) in Kingman, Arizona, a nursing informaticist’s primary responsibility is that of a liaison or even a translator between the Information Technology (IT) department and the bedside nurses. To better make the connection between these two areas of health care, the effective informatics nurse specialist will actually use the nursing process: Assessment, Planning, Implementation , and Evaluation . During the assessment phase, the nurse informaticists will observe a clinical area, interview the clinical nurses, and do a gap analysis. According to Savage (2010) this is where the existing systems are compared with what is needed. Salvage (2010) indicated the planning stage, involves obtaining buy-in from the director of nursing and other leadership of the facility (convincing the executives of the need for the system) and investigating regulatory requirements. Savage (2010) proffered it is vital for the informatics nurse specialist to be mindful of best practices and regulatory standards so that bad processes do not become automated. Once a need has been identified, the regulations have been researched, and the appropriate approvals have been obtained, the informatics nurse specialist may begin the implementation stage. This includes the composition of workflow diagrams which will cover every step of the implementation process. This procedure may begin with contacting various vendors and end with the actual installation of the system (Savage, 2010). Once the system is implemented, then the evaluative stage begins. Savage (2010) stated that this is where the “care and feeding” of a system begins. The informaticists may choose to begin with a “vanilla” version (a basic, bare-bones version) and then, after an evaluation, it is determined that an upgrade is needed to optimize its functionality. This is where the nursing process may begin all over again. The American Nurses Association® (ANA) agrees that the nursing informaticists can best function using the nursing process as a framework for practice and has put forth six Standards of Practice that incorporate these components. These standards “describe a competent level of nursing informatics ….The nursing process encompasses all significant actions taken by registered nurses and forms the foundation of the nurse’s decision-making” (ANA, 2008, p. 1). Standard 1 – Assessment The informatics nurse collects comprehensive data, information, and knowledge pertinent to the situation. Standard 2 – Problem and Issues Identification The informatics nurse analyzes the assessment data to determine the problems of issues. Standard 3 – Outcomes Identification The informatics nurse identifies expected outcomes for a plan individualized to the situation. Assignment: Topic 1 Nursing Informatics Standard 4 – Planning The informatics nurse develops a plan that prescribes strategies, alternatives, and recommendations to attain expected outcomes. Standard 5 – Implementation The informatics nurse implements the identified plan. Standard 6 – Evaluation The informatics nurse evaluates progress towards attainment of outcomes (ANA, 2008, p. 1). What Does an Informatics Nurse Need to Know? Ideally, a nurse informaticists is a master’s-prepared nurses who has obtained a special certification in Nursing Informatics from the American Nurses Credentialing Center® (ANCC). What does an informatics nurse need to know? It is true that technology plays a large part in the informaticists practice. For example, the nurse will need a working knowledge of applications such as conferencing tools, mind mapping, virtual worlds, interactive games, voice, video, graphics, and simulations. The informaticists will need to develop the skills necessary to integrate technology, communication devices, and information systems into the health care setting. The nurse will need to incorporate the elements of project management and be able to put into practice procedures that will preserve confidentiality and privacy. The nurse informaticists must be familiar be able to meet the competencies put forth by the American Nurses Association (ANA) and the Technology Informatics Guiding Education Reform® (TIGER) Initiative, among other organizations. “Nursing informatics participates in multidisciplinary strategic planning to determine system solutions that support patient care, set standards for clinical applications, conform to accreditation standards and regulatory requirements and transform clinical practice” (Cleveland Clinic, 2010, 3). What Does and Informatics Nurse Specialist Do? A nursing informaticists strives to take a wonderful profession and make it better. What nurse would not appreciate an improved system for the delivery of healthcare and streamlined, efficient documentation? An informatics nurse specialist can make it happen! “Nursing informatics results from integrating the triad of computer, information and nursing sciences” (Kaminski, 2010, 1). This triplet of expertise enables the nurse to manage and process data to support nurses in their practices. What is the primary task for a nurse informaticists? The goal is always to improve patient care. For example, there are several areas of clinical practice where the nurse relies on rote memory tasks. If a patient is admitted with certain physical conditions, for instance, the nurse needs to remember to be sure to order a physical therapy and an occupational therapy consult. Ideally, a system could be put in place that would automatically order the consults and take the nurse out of the loop. Why is this necessary? Savage (2010) indicated mistakes are most likely to happen when a nurse makes a cognitive shift, when the nurse’s attention is taken away from one task to another. By automating rote memory tasks, then the risk for nursing errors is diminished. If rote tasks can be streamlined, then the nurse is more able to focus on bedside patient care. Another area where patient care can be improved is to make information more portable. Special consideration must be taken is in the area of standardization. Savage (2010) stated that we must move toward systems that stay away from free text as much as possible. Ideally, the nurse should be able to chart his or her assessment, plan the patient’s care, and evaluate the interventions using a standardized nursing language. Nurses are already familiar with North American Nursing Diagnosis Association’s® (NANDA) list of approved nursing diagnoses. However, also available, but less familiar are the Nursing Interventions Classification® (NIC) and Nursing Outcomes Classification® (NOC) languages. Savage (2010) stated that these standardized lexicons are not only time saving, but also they ensure best practices and increase the transferability of a nurse’s documentation from one facility to another. To ensure these kinds of patient care standards, some health care facilities have provided for a fulltime staff Informatics Nurse Specialist to assist in using technology to work toward excellent patient care. Cleveland Clinic in Cleveland, Ohio, is a not-for-profit, teaching medical center and is a trailblazer in education, research, and the management of health information. The facility has begun an initiative that is entitled “The Nursing Unit of the Future” (Cleveland Clinic, 2010, 21) where an informatics team examines and assesses the benefits of various technological capabilities. “As the nurses evaluate the clinical efficacy of devices and applications, the Nursing Informatics team also evaluates technical and clinical outcomes criteria including: How devices withstand normal wear and tear How easily information is gathered and recorded User friendliness Clinician satisfaction Time efficiency versus existing methods Impact on patient safety Impact on patient satisfaction Impact on caregiver satisfaction” (Cleveland Clinic, 2010, 21).
