Concerns regarding the clinical question were further retrieved from the Cumulative Index to Nursing and Allied health Literature (CINAHL), where many scholars have conducted research and done literature reviews on the subject. CINAHL was chosen for background information as the database is extensive and many of the literature are peer reviewed and written by advanced nurse scholars.Good! Many experienced several admissions within a year, with chronic conditions resulting in multi-system failure. Often these directives have been overturned by family members who have difficulty in honoring patients wishes; or even by health care providers who for some reason failed to have proper system in place to ensure that such advance directive are visible in the patient medical records. This is an important issue as colleagues have expressed concerns regarding what can be done to ensure that patients wishes are honored as established in their advance directive. As a patient advocate, there seems to be some ambiguity surrounding why patient care settings allowed advance directives to be overruled.
