FINAL PICOT QUESTION 3 Final PICOT Question (Introduction -This should include background information, in this case- background information for the clinical question……) The initial question for this clinical situation was phrased due to concerns raised in my clinical environment and my role as a patient advocate. Concerns regarding the clinical question were further retrieved from the Cumulative Index to Nursing and Allied health Literature (CINAHL), where many scholars have conducted research and done literature reviews on the subject. CINAHL was chosen for background information as the database is extensive and many of the literature are peer reviewed and written by advanced nurse scholars.Good! (Title-always in bold letters, in the center) Why this is an important issue and the potential impacts on patient outcome Many patients admitted in hospital and more so in critical care units, are elderly adults between the ages of 65-98 years. Many experienced several admissions within a year, with chronic conditions resulting in multi-system failure. Assignment: Quality and Safety Competencies A significant amount of patients have initiated living will and advance directives prior to life threatening conditions. Citation? Often these directives have been overturned by family members who have difficulty in honoring patient’s wishes; or even by health care providers who for some reason failed to have proper system in place to ensure that such advance directive are visible in the patient medical records. This is an important issue as colleagues have expressed concerns regarding what can be done to ensure that patient’s wishes are honored as established in their advance directive. As a patient advocate, there seems to be some ambiguity surrounding why patient care settings allowed advance directives to be overruled. The lack of a clear consensus regarding who has the legal right to change a patient status when they are no longer able to make such decisions and FINAL PICOT QUESTION 4 advocate for themselves, have resulted in some poor patient outcomes with patients undergoing multiple diagnostic and treatment intervention without their consent. This has resulted in prolonged suffering, increase medical cost, and further complicate the decision making process in deciding when less is the best treatment option for some patients. Identify the type of question This is an intervention question as the dilemma often encountered, warrants the intervention of the health care team in making appropriate decisions and intervention that will allow autonomy, respect patient’s wishes, and assist family in coming to terms in accepting conditions of patients’ living will. Making diagnostic and medical decisions that will meet the desired outcome, and ultimately benefit the patient is an intervention. Assignment: Quality and Safety Competencies Literature review In analyzing a qualitative study conducted by (Crisp, 2007), the writer sought to determine why healthy adults execute advance directives prior to or in the early stages when they have life-threatening medical problems. Crisp (2007) indicated that in an age when medical technology and unwanted medical treatment have potentially made survival more fearful than death, society have been forced to reconsider concepts of death and dying. Though many families and health care providers encountered daily struggles in understanding how to facilitate and honor AD; Crisp (2007), sought to determine whether end-of-life decision making is improved by the use of formal ADs, whether patient outcomes are improved through the use of AD, and whether patient preferences as reflected in the AD represent important patient beliefs (Crisp, 2007). According to Crisp (2007), “whatever the outcome, the Patient Self-Determination Act (PSDA) passed by Congress in 1990 were founded on the principles of autonomy and selfdetermination” (p.180).
